Matvey Arye 48e0a61131 Remove triggers from chunk and chunk_constraint
Streamline code and remove triggers from chunk and
chunk_constraint. Lots of additional cleanup. Also removes need to CASCADE
hypertable drops (fixes #88).

This file creates function to intercept ddl commands and inject
our own logic to give the illusion that a hypertable is a single table.
Namely we do 2 things:
1) When an index on a hypertable is modified, those actions are mirrored
on chunk tables.
2) Drop hypertable commands are intercepted to clean up our own metadata tables.
